Important Reminders from the Readington Middle School Nurses:
The due date for FALL SPORTS FORMS is AUGUST 17th. Students who fail to turn in forms by the deadline will not be allowed to participate. All students who wish to try-out for fall sports must turn in a current physical exam using the state-mandated physical exam forms, a health history update (if the physical forms were completed prior to May 30th), the 2-page sports consent and, if applicable, treatment forms for asthma, severe allergies, and/or diabetes. Additionally, all 6th graders, as well as students new to RMS, must submit a current vaccine record showing receipt of Tdap and meningitis vaccines. All students with anaphylactic allergies and/or asthma must turn in treatment plans to the nurses each school year.
Fall sports include volleyball, girls soccer, boys soccer, cross country and field hockey. Try-outs begin August 29th. More information is available on the RMS Athletics webpage http://www.readington.k12.nj.us/page/541
School Nurse Debbie Nazzaro will only be in the RMS Health Office on Wednesdays and Thursdays from 9am-1pm through August 4th. The best way to reach the nurses during the summer is by email. Sports forms are reviewed as they are received and parents will be contacted by phone or email if the forms are incomplete. Forms should be handed in to the RMS Front office secretaries when the school is open (Mon-Thurs 8am-3pm) or may be placed in the mail box to the left of the front entrance doors when the school is closed. (The school is closed on Fridays during the summer.)

The office will be open from 7:00 a.m. to 3:00 p.m.
In the event of a delayed opening, school hours for students will begin at 9:35 a.m. (2 hour delay)
In the event of an early dismissal, students will be dismissed at 12:05 p.m. (lunch will not be served)
