Health Forms: 

All students NEW to Three Bridges School 

MUST provide: a completed Physical Exam Form  

& proof of Immunizations.

Universal Health Form

Epinephrine Order Form

Asthma Order Form

Medication Form (prescription or Over-the-Counter)

When to keep your child home:

• A temperature of 100 degrees or higher you child MUST remain home.  You may not medicate and send your child to school.

• Your child must be fever free for 24 hours without the use of fever reducing medications such as Advil or Tylenol.

• Vomiting and/or diarrhea -keep child home until 24 hours without any further vomiting or diarrhea.

• Any unusual rashes (should be evaluated by your health care provider.

• Treatment of bacterial conjunctivitis requires 24 hours of medication before returning to school.

• Strep Throat requires antibiotic treatment. Students must be on antibiotics for 24 hours before returning to school.

• Earaches should be evaluated by your health care provider.  

Cuándo mantener a su hijo/a en casa:

• Si tiene una temperatura de 100 grados o más, su hijo DEBE permanecer en casa. No puede medicar y enviar a su hija/o a la escuela.(Si su hijo/a tiene fiebre por 24 horas sin el uso de medicamentos para reducir la fiebre como Advil o Tylenol.

• Si su hijo/a vomitar y diarrea: mantenga al niño en casa hasta 24 horas sin más vómitos o diarrea.

• Cualquier brote o roja en su cerpa (debería evaluada con el medica o pediatría).

• Cualquier tratamiento de bacteria o conjuntivitis requiere 24 horas de medicamento antes de regresar a la escuela.

• Dolor de garganta (faringitis estreptocócica) requiere un tratamiento de antibióticos. Su hijo/a debería tomar durante 24 horas el medicamento antes de regresar a la escuela.

• Dolor de oído debería ser evaluado por el doctor o su pediatra.
