    Click on the links to the left for required sports forms and helpful information. Sports forms must be turned into the RMS Health Office. During the summer, forms can be placed in the mailbox at the Main Entrance to RMS. Contact the nurses with any questions regarding sports physical forms. Students who fail to submit required forms by the due date (see LAST DAY dates above) will NOT be allowed to try out.

    Students with epinephrine orders, diabetes, and asthma must submit yearly management forms from their health provider available under the Forms link at the left of this page. Students with these conditions MUST submit these orders PRIOR TO TRY-OUTS to be approved for sports participation.  Students should have their own inhaler, epinephrine autoinjector, and emergency diabetic supplies with them at all practices and games. 

    In-coming 6th Grade students must submit a vaccine record showing receipt of the Tdap and meningitis vaccines or documentation of a medical or religious exemption, if applicable.
