REMINDER TO 6TH GRADE: In-coming 6th Grade students must submit a vaccine record showing receipt of the Tdap and meningitis vaccines or documentation of a medical or religious exemption, if applicable.
General Health Update - complete and return this form to your child's school on the first day of school
Medication Administration of Prescription (RX) or Over-the-Counter (OTC) Meds This form authorizes the school nurse to adminster medication to a student on a daily basis (such as medicine for ADHD or diabetes) or on an "as needed" basis for minor ailments (such as Advil or Tylenol for headaches or menstrual cramps, or seasonal allergy meds such as Claritan or Allegra) if needed at school. This form must be filled out by a health provider and signed by the parent/guardian for each medication to be given at school. Different forms are required for asthma medication and epinephrine (refer to Health Office webpage)
Students with epinephrine orders, diabetes, and asthma must submit yearly management forms from their health provider available under the Forms link on the Health Office webpage. Students with these conditions MUST submit these orders PRIOR TO TRY-OUTS to be approved for sports participation. Students should have their own inhaler, epinephrine autoinjector, and emergency diabetic supplies with them at all practices and games.
The Pickup Authorization Form for all students is due back the first day of school, please remember to send it in with your child.
The deadline for payment for the 8th Grade Philadelphia trip has been extended to September 18. Click here for important information. All money is handled directly through the company, so please make sure you meet the necessary deadline.
Schedules are available in Genesis. *Important – Schedules will remain locked until all mandatory forms (Photo Form, Acceptable Use and Behavior Contract) are completed in Genesis.

The Readington H.S.A (our district’s PTO) welcomes you to another school year!
Invest In Your Child – We are kicking off our primary fundraiser, Invest In Your Child! 100% of funds raised will go to supporting our Readington Schools and you will not be asked to do any additional fundraising for the H.S.A. It is $45 per family due by October 1st. We prefer online donations at readingtonhsa.ptboard.com but you can also donate via check. Many companies will do matching donations, and we appreciate if you check with your employer!
Readington H.S.A. Goes Digital! – Join us on PTBoard, our new digital platform, for all things H.S.A. including volunteer opportunities, our calendar, announcements and reminders, Invest In Your Child, and our district directory. Families that were included in the directory last year should have gotten an email invitation to join us on our PTBoard site. Please check your junk/spam folders. Parent Drop Off Procedures: On the first day of school, September 5th , all vehicles involved in either school business or student drop-off/pick-up, please use the entrance off Readington Road that comes to the front of the school. The Holland Brook access road to enter the middle school will be reserved for buses only. This helps to promote a safer and more effective drop off of students.
